Community Health Worker (CHW) Responsibilities & Duties

• Conduct home visits to educate community members on health practices and resources
• Assist individuals in managing chronic conditions and accessing healthcare services
• Provide culturally appropriate health education and information
• Support community members in navigating the healthcare system
• Advocate for individual and community health needs
• Collect data and relay information to healthcare providers to inform care
• Develop and implement community outreach programs
• Facilitate access to social services and other community resources
• Organize and participate in health screenings and immunization clinics
• Collaborate with healthcare providers and community organizations
• Build trusting relationships with community members
• Empower individuals to adopt healthy behaviors and lifestyle changes

Community Health Worker (CHW) Qualifications & Skills

• Previous experience in health education or community outreach
• Strong knowledge of community resources and services
• Ability to work with diverse populations
• Excellent communication and interpersonal skills
• Training in motivational interviewing
• Experience in data collection and reporting
• Background in public health or social work
• High school diploma or GED
• Strong organizational and problem-solving skills
• Ability to work independently and as part of a team
• Basic computer skills, including familiarity with Microsoft Office
• Valid driver's license and reliable transportation
• Comfortable working in various community settings
• Demonstrated commitment to improving community health
• Effective communication skills
• Cultural competency and sensitivity

Position is subject to successful completion of a drug screen and background check. 

Position is contingent upon grant award.
